Journal Templates

Structured daily journaling supports shared decision-making by surface patterns your provider may otherwise overlook.

Daily journaling doesn’t require stream-of-consciousness writing. A simple template with time-stamped entries—such as mood (scale 1–5), any notable events, physical symptoms, and medication taken—builds a clearer picture than alone. Clinicians may notice, for example, that irritability spikes consistently after missed meals or social interaction, suggesting patterns beyond pure mood disorder.

Some templates include prompts for thought records: what happened, automatic thoughts, evidence for/against them, and alternative views. This isn’t therapy—just awareness. The value lies in capturing moments when feelings were intense, not perfect entries every day. Even 3–4 entries weekly can reveal trends when reviewed over two weeks.

Use these logs not to justify your experience but to help your provider see how symptoms ebb and flow in real life. For instance, tracking sleep onset, duration, and next-day energy may support decisions about timing or type of intervention. Sharing a journal isn’t mandatory, but many patients find it builds trust and clarifies what to explore in session.
